New and transferring patients!  Save time at patient check-in by filling out the following three forms before your appointment. You can fill them out on your computer, then print them out to bring to your first appointment. You may also print the forms to fill out later by hand if you wish. (*Note: Adobe® Reader is required to open and fill out these forms. Free for Mac or PC at Adobe.com)

(1) Transferring Your Medical Records: Use our Authorized Request for Release of Protected Health Information (PHI) form to request transfer of your medical records from another physician or medical center. After you fill in and print out this form, you can fax or bring it to your current physician to have your records transferred to our practice. There may be a fee for this. (2) Patient Intake Form: Be sure to fill out our Patient Intake Form before you come in. Please fill in as much information as you can, then print out and bring the form with you to your first appointment.

(3) Patient Privacy Policy: In keeping with the patient privacy standards set forth by the Health Insurance Portability and Accountability Act of 1996 (HIPAA), we have established our HIPAA Notice of Privacy Practices document which can be read here. After reading this document, please fill in and print our HIPAA Acknowledgement Form to bring with you to your appointment.

Dr. Teresa Tam is a graduate of the Rosalind Franklin Chicago Medical School, where she received the Dean’s Award at her graduation (1994). She undertook her residency in 1994 at St. Joseph Hospital, completing the program in 1998. She has been in private practice since then. She has also been affiliated with Presence Saint Joseph Hospital as the Associate Director of their Obstetrics & Gynecology residency program, as well as an on-staff attending physician. She has been an attending on staff at Northwestern Memorial Hospital for the past 12 years. She is Board certified by the American College of Obstetricians and Gynecologists, and a fully licensed physician and surgeon by the State of Illinois (license no.: 036.093565).

Along with her medical practice, Dr. Tam is also an accomplished medical researcher having been published in a number of professional medical journals. She is a noted presenter at national and international medical conferences having presented over two dozen oral presentations, abstracts, video, and poster presentations combined. To date, she has written over 13 published research papers and produced a series of medical education videos. She was awarded “Attending of the Year and received the “National Faculty Award“, an unprecedented SIX times as an attending physician and Associate Director of the Ob/Gyne residency program at Presence Saint Joseph Hospital in Chicago, and as an attending physician and mentor at Presence Saint Francis Hospital in Evanston.

In 2012, Dr. Tam took a sabbatical from her private practice to undertake a fellowship program in Minimally Invasive Gynecological Surgery (MIGS) at Penn State Hershey Medical Center in Hershey, Pennsylvania. She left Chicago and moved to Hershey, PA for one year. There, she received advanced training in laparoscopic surgery and robotically-assisted surgery using the da Vinci® Robotic Surgical System. She completed her fellowship in June 2013 and returned to Chicago to start her new solo practice, “All for Women Healthcare, SC“, located in the Stone Building at 2800 N. Sheridan Road, Suite 205N, in Chicago.

In February 2017, thanks to a growing demand for her medical expertise, Dr. Tam moved to a larger, completely new office in the Presence Center for Advanced Care (PCAC) building located at 2845 N. Sheridan Road, Suite 902, in Chicago, just down the block from her old office in the Stone Building. The PCAC is a truly modern, state-of-the-art medical building having just been completed in 2015. Dr. Tam’s new office offers patients a beautiful waiting area and four new exam rooms. The PCAC also offers visitors and patients the convenience of valet or indoor self-parking, and even a Walgreens pharmacy located in the lobby.

Besides medicine, Dr. Tam’s other passion is running… a lot of running! Over the past 20+ years, she has run in no less than 28 marathons (at 26.2 miles each!). She has completed full marathons all over the U.S. and the world including the Chicago Marathon (5 times, latest in October 2015), Las Vegas, Disney World, New York, Washington, DC, Rome, and Paris, just to name a few. She was featured in the news article, “50 Chicago Women in Sports“, appearing in Windy City Sports – Chicago’s Fitness & Recreation Guide Magazine, Chicago, Illinois, September, 1997Dr. Tam fully understands the needs of female athletes and has helped many face special health challenges unique to their athletic lifestyle. It is no surprise that many of Dr. Tam’s patients are athletes, too. They love the fact that their doctor can identify with their unique needs as women athletes.

In all, Dr. Tam shows genuine understanding, concern, and compassion for all her patients. She takes the time to listen to their needs, as well as advise and educate them.
